DISCHARGE PROGRAM

A Community Liason

El Norte pharmacy has a goal of integrating knowledgeable pharmacy into discharge prescription services to help improve timely discharge prescription processing, bedside or at home medication delivery, promotion of patient access to medications through prior authorization assistance, and resolution of third party adjudication issues. Our purpose is to assist case managers and other members of the interdisciplinary health care team in ensuring safe and effective care transitions and patient satisfaction throughout the discharge process.
